Week one Homework:  please bring in completed WCB pink copy, give your supervisor my business card, prepare a map to your placement including driving and bus route from Algonquin's Woodroffe campus and include your supervisor's name and phone number, and complete your log entries for this week.  Log is due on Monday to me or to Phyllis for my mailbox.  Good luck everyone!

Week two Homework:  Did you bring in WCB form?  Did you give the business card to your supervisor?  Did you hand in your map to Lisa?

Log entry info for this week includes:  overview of the two days, two things that challenged you, two things that you succeeded or excelled in, and list what you did for self care this week.

Remember, logs should be handed in on Mondays to Lisa in Crisis Intervention or to Phyllis.  Please put your name on the front of your log.

Week three Homework:  In your log, write your agency's mission statement, your comments on it, two examples of how you observed it being put into action this week.  Continue with your overview of the two days, two things that challenged you, two things that you succeeded or excelled in, and list what you did for self care this week.

Please work on your goal setting that we discussed this week.  Set your goal, how you will work to meet the goal, and how you evaluate your progress in working toward the goal.
